New Patient Experience

Thank you for choosing Comprehensive Women’s Care of Columbus, PC.  We look forward to providing you with professional health care in a friendly and welcoming environment.  This page is designed to provide you with important information that most of our patients find valuable.  Please take a moment to read through this information.  If you have any questions, feel free to contact us during regular office hours by phone at (706) 341-3311 or by email.  We will return your emails during office hours. 

 

YOUR FIRST VISIT

As a new patient to our practice you will be required to complete our New Patient Paperwork.  You will be sent our new patient paperwork once you have been scheduled for an appointment. This paperwork will be due within 3 days of scheduling your appointment.  This allows us to expedite your time in the office by preparing your electronic medical record before your visit.  You will not need to print or email/fax anything.  The whole process can be completed from a computer, smartphone or iPad.

Our office policy requires that new patients must submit your completed paperwork within 3 days of scheduling your appointment.  If not received within the required timeframe, your appointment will automatically be cancelled and will need to be rescheduled.  We appreciate your cooperation with this policy.

WHAT TO BRING

  • Current insurance card

  • Photo identification card

  • Method of payment for copay, if applicable (Cash, Credit Card)  All copays are collected at the time of visit.

If you have questions regarding your financial responsibility you can call the office before your appointment to confirm.

  • Names and dosages of all medications, prescription and over the counter

  • Names and contact information for all other providers (Primary Care, Previous OB/GYN, etc.)

  • Any records relevant to your appointment
